Payment Total Cost - $3000.00 Fees include all classes, instruction, books and materials. Payment Plan ~ $500.00 non refundable reservation due with acceptance of application ~ $2500.00 due at start of module 1 Refund Policy ~ The $500.00 reservation fee is non-refundable. ~ The $2500.00 fee is fully refundable up to 5 days before the start of training. ~ Within 5 days of the start of training, there are no refunds. Disclosure and Acceptance of Terms By submitting this form I hereby declare the above information is true to the best of my knowledge. I understand that misrepresentation of this information constitutes grounds for the rejection of this application, expulsion from the program and revocation of certification.
